No. The evaluation is free, every feature, no time limit. Buying removes the purchase reminder and keeps updates coming.
Not from Nod. Ask-about-code is optional and bring-your-own-key: the app talks to your provider directly, and whatever that costs is between you and them. Nothing is resold on top of the license.
Polar, the merchant of record. Your payment details never reach Nod, and the receipt comes from Polar.

Yes. Checkout signs in with GitHub for now, so email me and I will set your license up by hand until GitLab checkout lands.
Yes. The license permits internal use at any company, commercial or not. What it forbids is building a competing product from the source.
Per person. Your GitHub account is the license, and Nod activates on any machine you sign in on. Machines are not counted.
